Transcript Request Procedures
For Students Formerly Enrolled in Gwinnett County Public Schools (GCPS)
Please use the Transcript Request Form above to request a transcript.
THERE IS A $2 FEE PER TRANSCRIPT.
Cash or money orders are accepted.
In order to process requests for transcripts/records, we will need the following information:
- Your complete name at the time you attended a Gwinnett school (maiden name, if applicable).
- Your date of birth.
- The last Gwinnett County school you attended.
- The last year you attended a Gwinnett County school or your graduation date. Please state whether you graduated.
- Your daytime phone number.
- An address to which the transcript should be mailed, if applicable.
- Your signature MUST appear on the transcript request form. For students under age 18, a parent or guardian must sign the form.
- A copy of your photo ID.
- Please include the $2 fee per transcript requested. We accept cash or money orders.
Please note: The Office of Student Records accepts inactive files three years following a student’s last attendance at a Gwinnett County Public School. Therefore, if the student whose records you are attempting to retrieve last attended a Gwinnett County Public School three years ago or less, please contact the last school attended directly. Only the person whose record is being requested may sign the Transcript Request form. For students under age 18, a parent or guardian must sign the form.
You may print the Transcript Request form and send a copy of the completed form via mail, fax, or hand-delivery. Please refer to the three options mentioned below.
We will attempt to process your request within two business days. However, there may be exceptions to stated time frame. If the form is incomplete, or has not been signed, the transcript request cannot be processed.
To comply with Family Educational Rights and Privacy Act (FERPA) regulations, we must obtain a signature before we can process a transcript request. We have provided the following three options:
- Walk-Ins - Please see the receptionist at the Instructional Support Center (click here for directions) for a Transcript Request Form. Please complete the form and leave it and your payment with the receptionist. Your request will be processed within two business days.
- Letter - Mail your request with your $2 fee per transcript and a copy of your photo ID to the following address:

- FAX - All fax requests should be sent to 678-301-6323. You may pay the $2 fee per transcript and show your photo ID when you pick up your transcript from the Instructional Support Center.
We will accept transcript requests over the phone; however, the transcript must be picked up in person. A copy of your photo ID will be requested when you pick up your records. The phone number for the Office of Student Records is 678-301-6144. You may leave a message that should include your daytime phone number and the date and time you would like to pick up your transcript, along with all relevant information. The $2 fee will be required when the transcript is picked up.
